  • Book Online: Click on any of the Book Now icons on our website to book online.

    Please make note of your user name and password once you make an online booking account, this will allow you to rebook efficiently.

    We routinely check our voice mail. We have weekly reception support, hours are variable. Please leave a message if you need any assistance.

    • Cancel Online: Outside of 24 hours, sign into your Booking Account with your user name and password. Access your booked appointments and cancel accordingly.

    • Cancel by Phone: Please call 604-708-4762.

    • 24 Hour Cancellation Policy: Our clinic adheres to a 24 hour cancellation policy. You will be charged the full fee for your appointment if sufficient online/phone cancellation is not received.

    • NOTE: We do not reply to the automatic reminders that our scheduling system emails out. Replying to this email does not confirm a cancellation.

  • Form Physiotherapy does not currently bill directly to your extended healthcare provider. We will print you a detailed receipt after your appointment to submit to your extended health plan.

  • Business Hours:

    • Monday to Friday, 7am to 7pm

    • Reception hours: variable

    • Our voice mail is checked frequently throughout the day.

    • Please arrive 10 minutes early for your first visit to allow time for you to find us and change into any required clothing.

    • Bring gym shorts/loose tights if we will be assessing a lower body or low back problem. For an upper extremity, upper back or neck problem, a sports bra/shirt removal may be required.

    • Please note your MSP/PHN number is only required if you are being seen for an ICBC claim or are on income assistance. Under the latter circumstance, the government will co-pay a portion of your session.

    • One of the benefits of our one-on-one practice is that we are able to remain on time for your appointments. As such, we ask you to arrive a little ahead of your treatment session to make full use of that time.

  • The government MSP copay is available on a limited basis to BC Residents on income assistance. We will be able to verify if you qualify for this at your first visit.

    We require your PHN/Care Card # and birthdate in order to for us to verify your coverage.

  • Telehealth Physiotherapy is the delivery of physiotherapy services through electronic communication, like a video or by telephone. Telehealth Physiotherapy can be very beneficial for clients who are unable to attend in-person sessions but require a continuation in care, for example going on vacation, an illness, or a lack of mobility due to an injury.

  • Payments can be made by cash, Visa, Mastercard, or debit.

    We will sanitize machines after each use.

    • We are located at U16 – 601 West Broadway, Vancouver, in the Broadway Plaza building complex. The complex is situated on the NW corner of Broadway and Ash, one block west of Cambie St.

    • Our unit is located on Broadway at street level on the south side of the plaza complex, several steps past Tractor. We are not located in the tower of our complex.

    Parking:

    • Street Parking: Metered parking is available on Ash, 8th Avenue, and during certain hours on West Broadway. Rates vary as per location.

    • NOTE: during Translink construction, there is significantly less street parking available. Plan accordingly.

    • Underground Parking: There are several underground lots in the vicinity, including at the Echelon Centre ($2/hr), Whole Foods ($3/hr) and directly under our building ($5/hr). An above-ground lot is available at the Cactus Club across Ash St. from us at $3 /hr.

    Public Transit:

    • Canada Line Sky Train: We are 1 block west of the Broadway/City Hall Station or 4 blocks south of the Olympic Village station.

    • Bus: There are many buses that run regularly along Broadway Street, West 4th/6th, Cambie Street, and Oak Street, which are all within walking distance of our clinic. We do have a bus stop located directly in front of our plaza complex on Broadway. (please check www.translink.ca for specific schedule information).

  • Form Physiotherapy, in accordance with the guidelines set out by the BC Government, BCCDC, the Physiotherapy College and Association, has developed protocols and procedures to manage COVID-19 and to ensure the safest possible environments for our valued patients and staff.

    COVID 19 Wellness Checklist – please ensure that you have NONE of the following:

    • Fever, cough, runny nose, sore throat.

    • In the past 14 days you have not been in contact with someone who has a presumed or confirmed case of COVID-19 or Acute Respiratory illness.

    • In the unlikely event that you develop these symptoms, late cancellation fees will be waived.

    • Please bring your own Face Mask. Patients and staff all need to wear a mask.

    • Social distancing, hand-washing, and practicing proper coughing/sneezing etiquette will be crucial.

    • It is also a good idea to avoid touching high-touch surfaces such as light switches, counter tops, and door handles whenever possible.

    • Please don’t arrive too early for your appointment. We will be trying to stagger the number of people in the waiting room at one time.

    • Do not bring other people with you to your appointment unless help is needed with mobility, with language interpretation or the patient is under the age of 16. absolutely necessary.

    • If someone else drives you to your appointment and doesn’t need to come in with you, please ask them to wait in the car.

  • As per the provincial recommendations, masks are now optional in clinic. Please let us know if you would prefer we wear a mask, and we would be happy to do so.
